State Health Plan Member ID cards have been redesigned for all members on the 70/30 and 80/20 Plans. The state has begun mailing new State Health Plan ID Cards for the 2019 plan year; all members should receive their cards by the end of December 2018 which will reflect the plan choice you made during Annual Enrollment. The new cards have a new look to provide greater transparency about your benefits as well as better descriptions of services and required copay amounts.
New details are included on the ID Card, including available services, copays and other details. Please take a minute to review your redesigned ID card when it arrives in the mail, and provide it to your medical providers beginning January 1, 2019.
Access Blue Connect to Request Replacement Cards
Throughout the year, you may access the Blue Connect system to request replacement health insurance ID cards.You can access Blue Connect by logging into the MY Pack Portal> Employee Self Service> Benefit Details> Enroll in Benefits. This path will take you to the State’s enrollment system, the same system in which you completed Annual Enrollment.
Once logged in click on Blue Connect in the Quick Links section of the portal (left side of home screen). Once in Blue Connect you can request new ID cards, access wellness benefits, view claims information, find a provider and more!
